  2. Enroll your patient(s) by uploading their information into the Mozzaz Platform and providing the specific connected-health devices they should receive. Depending on your requirements Mozzaz can take care of outreach, patient education and walk patients through taking their first reading(s).

  3. Mozzaz will quickly provision and ship devices to either the patient’s home or your office as determined

  4. Review device data easily within the Mozzaz Portal for clinicians, or in the Mozzaz Patient App for patients. Staff are notified if a determined reading enters a critical range in addition to patients with what next steps should be taken. These are to be determined by the specific care requirements to the patient population and set out clearly in implementation.
