Mozzaz to Showcase RPM and Digital Health solutions at North Carolina Primary Care Association Annual Board Meeting
Mozzaz, a leading remote patient monitoring (RPM) & patient engagement solution provider announced their participation in the annual North Carolina Community Health Center Association’s (NCCHCA’s) Primary Care Conference and Board Meeting. The conference takes place from June 7th – 9th and aims to bring together leaders from both Rural Health Centers (RHCs) and Federally Qualified Health Centers (FQHCs) to discuss the latest information and operating strategies as they relate to care in North Carolina.

Mozzaz’s is excited to share their successful RPM implementation with High Country Community Health (HCCH), where they implemented a customized RPM program for rural patients in NC with hypertension and diabetes. Mozzaz worked with the High Country team to design and implement a successful RPM program tailored to their patient population residing in rural locations with connectivity limitations. Utilizing cellular devices and SMS patient engagement were some of the effective strategies that were implemented.
Patients were equipped with a mix of cellular and Bluetooth blood pressure and glucometer devices from Mozzaz’s extensive integration directory and connected to the Mozzaz Platform in-office and via Mozzaz’s concierge services. Through utilizing the Mozzaz Platform, members of the program achieved improved readings-in-range for blood pressure by over 50%.
