A Patient Centered Approach to Surgical Recovery

In a recent update HCA Healthcare Inc. has announced the official enterprise-wide roll out of their Enhanced Surgical Recovery (ESR) digital-health program. This implementation comes off the back of their 20 years of clinical research, a multitude of peer reviewed publications and an intensive review of internal data to make a multidisciplinary approach to pre and post operative care.

ESR utilizes specific pre-, intra-, and post-operative care interventions which are designed to improve the patient experience and more importantly outcomes. The program consists of six key-areas inspired by the Enhanced Recovery after Surgery (ERAS) program which has been standard practice in Europe for many years. HCA Healthcare’s ESR program is focused on the below areas which will have the greatest impact on patient outcomes.

  1. Goal-directed fluid therapy: active patient monitoring of fluid balance to ensure optimal hydration level throughout surgery

  2. Patient education: ensuring patients are actively taking in a role in their care by providing a personal recovery checklist that they can follow

  3. Multimodal pain management: taking an individualized approach to pain by instructing opioids to only be used as needed, and instead replacing them with non-opioid pain relievers and other pain management techniques

  4. Permissive pre-op hydration/carb loading: making sure that the patient has consumed enough fluids and a carbohydrate-rich drink prior to surgery with the goal of improving hydration and insulin resistance while curbing nausea symptoms.

  5. DREAM = drink, eating and mobilization within 24 hours post-operation: as described, encouraging patients to eat, drink and move shortly after their surgery

  6. Multidisciplinary team: consists of creating a group of team members, such as a surgeon, anesthesiologist, nursing lead and pharmacy which will work together to guide the facility’s program and each patient’s unique care journey.  

HCA’s ESR program draws from 35 million annual patient encounters to inform best practices and continuously improve provided care. Based on data collected from 141,482 joint replacement, gynecologic oncology, colorectal and bariatric surgeries in 2021, the ESR program has statistically proven to improve patient outcomes by the below mentioned amounts:

  • A 2.1 day reduction in length of stay;

  • A 54% decrease in 30-day readmissions for joint replacements;

  • A 45% decrease in 90-day readmissions for gynecological surgery;

  • Up to a 44% decrease in opiod usage; and

  • 2.4 million few morphine medications dispensed
